Abstract

Background: To suggest measures to prevent burnout of physicians during the coronavirus disease 2019 (COVID-19) pandemic, we conducted a systematic review of research on physician burnout due to COVID-19.
Methods: Three foreign databases were used: MEDLINE, Embase, and the Cochrane Library. A search was performed using the following search terms: COVID-19, coronavirus, NCOV, physicians, doctors, residents, and burnout. Four rounds of review were performed to enhance the consistency of quality assessment. The Joanna Briggs Institute checklist for analytical cross-sectional studies was used to assess the quality of the eligible studies.
Results: A total of 465 studies were identified, of which 32 were selected. The quality assessment determined that 19 studies met the Joanna Briggs Institute criteria and had a low risk of bias. The most common tool used to measure physician burnout in the 32 studies was the Maslach Burnout Inventory.
Conclusion: This study systematically evaluated the literature on physician burnout. However, the generalizability of our findings to Korean physicians may be limited. The composition of the medical personnel in Korea, such as open doctors, volunteer doctors, medical professors, and public health doctors, differs significantly from that of other countries. Nevertheless, evaluating the current status of physician burnout due to COVID-19 globally may hold meaningful implications for policymaking.

이 논문은 코로나19 유행의 지속에 따라 의료진이 겪을 피로감과 소진(번아웃)에 대한 연구 현황을 확인하고, 이를 바탕으로 소진 예방관리를 위한 대안을 만들기 위하여 수행된 연구 결과를 체계적 문헌 고찰 방법을 활용하여 정리한 논문이다. 다양한 국가에서 다양한 전문과 의사의 소진에 관한 연구가 이루어졌으나 대부분 초기 단계 연구이었다. 기존의 메르스나 사스 등의 사례로 미루어보았을 때, 코로나19 사태도 의료진의 소진이 동반될 것이라는 가설하에 한국에서 체계적 문헌 고찰 연구를 진행한 결과라는 점에서 상당한 의미가 있다. 지속되는 코로나19 상황에서 국내 의료진의 소진이 국민의 생명 보호와 방역 유지에 중요한 문제로 인식되는 가운데, 이 논문은 의료진 소진 해소를 위한 실질적인 정책 개발 연구의 기초자료를 제공할 것으로 판단된다.

서론

코로나바이러스감염증-19 (코로나19)는 2019년 12월 중국 우한에서 처음 발견된 이후 전 세계적으로 유행하고 있으며, 세계보건기구는 2020년 3월 11일 팬데믹(pandemic)을 선언하였다. 우리나라 정부는 코로나19 집단감염과 산발적 지역 확산이 시작된 2020년 2월 23일부터 감염병 위기경보 단계를 ‘심각’으로 상향하여 대응해오고 있다[1]. 전 세계적으로 코로나19 환자는 2021년 5월 23일자로 1억 6,634만 명을 넘어섰고, 사망자수는 344만 명에 이른다. 국내에서는 누적 확진자수 136,000여 명, 사망자 1,934명을 기록하였다[2,3].

코로나19는 무증상, 마른기침과 발열, 호흡곤란 등의 호흡기 질환을 유발하며, 중증으로 진행될 경우 사망에까지 이를 수 있는 특징이 있다[4,5]. 감염병 유행시기에 의료종사자는 스트레스로 인한 정신건강 문제, 번아웃(burnout) 등을 겪을 수 있는 것으로 보고되었으며, 높은 감염률 및 사망률은 의료종사자의 불안과 스트레스를 유발하는 요인이다[4,6].

Freudenberger [7]는 번아웃을 직업과 관련된 만성적인 스트레스에 노출되어 발생할 수 있는 심리적인 증후군으로 정의하였으며, Zakaria 등[8]은 신체적 징후를 피로, 두통, 위장 장애, 불면증 등으로 설명하였다. Maslach와 Jackson [9]은 Maslach Burnout Inventory (MBI)를 개발하며, 번아웃을 정서적 소진(emotional exhaustion), 비인격화(depersonalization), 개인적 성취감(personal accomplishment)의 세 가지 측면에서 정의하였다.

국내에 중증급성호흡증후군(severe acute respiratory syndrome)과 중동호흡기증후군(Middle East respiratory syndrome)이 발생하였을 때, 해당 감염병 대응에 종사하는 의료종사자의 30% 이상이 번아웃을 경험한 것으로 보고된 바 있다[10]. 최근 코로나19 대응 의료종사자는 높은 우울과 불안을 느끼고 있으며, 수면의 질도 낮다고 보고되었다[11,12]. 코로나19로 인한 의사의 번아웃을 조사한 연구들에서 의사의 약 40% 이상이 번아웃 증상을 겪고 있다고 보고하였으며, 특히 전공의의 경우에는 20%부터 60% 이상까지 번아웃 증상이 다양하게 보고되었다[13-17]. 코로나19 대유행은 감염병의 전파기간이 길고 현재진행중이기 때문에 감염병 대응에 종사하는 의료종사자의 번아웃 악화에 잠재적으로 더 심각한 영향을 미칠 수 있다.

이 연구는 코로나19가 의사인력의 번아웃에 미치는 영향에 관한 문헌을 종합하고 체계적으로 분석하여 현황을 파악하고, 나아가 향후 필요한 연구방향을 제시하고자 한다. 이를 바탕으로 감염병으로 인해 발생할 수 있는 공중보건위기 상황에서 의사인력의 번아웃을 예방하기 위한 근거자료를 마련하는 데 기여하고자 한다.

이 연구의 목적은 코로나19로 인한 의사의 번아웃에 대한 선행연구를 체계적으로 고찰함으로써 코로나19 상황에서 방역 최일선에 있는 의사의 번아웃을 예방하기 위한 기초자료를 제공하는 것으로 구체적인 목적은 아래와 같다. 첫째, 코로나19가 감염병 대응 의사의 번아웃에 미치는 영향과 관련한 문헌을 체계적으로 고찰한다. 둘째, 번아웃 측정도구에 따른 코로나19 전파와 감염병 대응 의사의 번아웃 간 관련성을 파악한다.

방법

1. 연구설계

이 연구는 코로나19가 감염병 대응 의사의 번아웃에 미치는 영향에 대한 체계적 문헌고찰 연구이다.

2. 문헌 선정기준과 배제기준

문헌 선정기준은 연구대상이 의사이고, 번아웃의 원인이 코로나19인 연구이다. 번아웃에 대한 결과를 통계값으로 제시한 연구를 대상으로 포함하였다. 문헌 배제기준은 연구대상이 의사가 아닌 연구, 질적 연구, 초록만 제시되었거나 번아웃 관련 결과값이 제시되지 않은 연구이다.

3. 문헌 검색 및 최종 선정 과정

본 연구는 PRISMA (Preferred reporting items for systematic reviews and meta-analysis) [18]에 의거하여 수행하였다. 문헌 검색은 2021년 4월 26일 이루어졌으며, 현재까지 국내에서는 코로나19가 감염병 대응 의사의 번아웃에 미치는 영향을 다룬 체계적 문헌고찰 연구가 보고된 바 없다. 문헌 검색은 국외 학술지에 게재된 문헌을 대상으로 하였으며, MEDLINE, Embase, Cochrane Library의 3개 데이터베이스를 활용하였다. 검색식은 ‘COVID 19’, ‘coronavirus’, ‘NCOV’, ‘physicians’, ‘doctors’, ‘residents’, ‘burnout’ 검색어를 조합하여 구성하였다.

4. 문헌 질 평가

질 평가는 두 명의 연구자(MJK, JHP)가 수행하였고, 질 평가의 일치도를 높이기 위하여 4회의 의견 수렴과정을 거쳤다. Joanna Briggs Institute의 checklist for analytical cross sectional studies 도구로 평가하였다[19]. 이 도구의 평가항목은 대상자 선정기준, 대상자에 대한 기술, 질병 위험요인 노출 측정방법, 측정의 객관성, 교란변수 정의, 교란변수 통제방법, 결과변수측정, 적절한 통계분석방법 수행 등 8개로 구성되어 있다. 각 항목에 대하여 비뚤림에 대한 위험이 높음(high risk), 비뚤림에 대한 위험이 낮음(low risk), 비뚤림에 대한 위험이 불확실(unclear risk)로 평가하였다.

결과

1. 문헌의 일반적 특성

최초 수집된 문헌은 총 465건이었으며, 중복문헌을 제거하여 328건의 문헌이 선택되었다. 이후 수집된 문헌의 제목과 초록을 검토하여 본 연구의 목적에 맞는 문헌 83건을 선택하였다. 문헌 선택의 전 과정은 두 명의 연구자(MJK, JHP)에 의해 독립적으로 수행되었으며, 연구자 간 충분한 논의를 통하여 문헌 선택의 기준을 일치시켜 최종적으로 32건을 선정하였다(Figure 1).

연구가 진행된 국가는 한국 1건, 미국 6건, 프랑스 1건, 3개국(영국, 폴란드, 싱가포르) 1건, 4개국(미국, 중국, 사우디아라비아, 대만) 1건, 네덜란드 1건, 아일랜드 1건, 파키스탄 1건, 터키 2건, 브라질 1건, 캐나다 1건, 스페인 1건, 이탈리아 2건, 벨기에 2건, 이집트 3건, 리비아 1건, 중국 1건, 국가를 알 수 없는 문헌은 5건이었다.

연구대상자인 의사 전공별로 internal medicine 3건, emergency medicine 2건, otolaryngology 1건, neurosurgery 2건, paediatric medicine 1건, palliative medicine 1건, 7개 전공과목(medical oncology, radiation oncology, surgery, plastic surgery, pathology, genetics, radiology) 1건, 의사의 전문과목을 표기되지 않은 연구가 21건이었다.

의사의 번아웃을 측정하기 위해서 (1) Shirom-Melamed Burnout Measure, (2) MBI, (3) MBI-Human Services Survey for Medical Personnel, (4) Cophenhagen Burnout Inventory, (5) MBI-Human Services Survey, (6) Oldenburg Burnout Inventory, (7) Professional Quality of Live Measure 5-compassion fatigue, (8) Professional Fulfillment Index, (9) MBI-General Survey, (10) Physician Work Life Study Item, (11) Utrecht Burnout Scale, (12) Mini-Z Burnout Assessment, (13) a brief measure of coronavirus disease 2019 (COVID-19) related burnout symptoms, (14) non-proprietary single-item measurement와 같이 14종류의 측정도구가 사용되었다(Table 1) [13,16,20-49].

2. 질 평가 결과

최종 선정된 문헌 32건에 대한 질 평가를 시행한 결과 충분한 설명이 기재되어 비뚤림 위험이 낮다고 평가한 연구는 19건이었다. 교란변수에 대해 언급하지 않은 10건에 대해서는 비뚤림 위험이 높다고 평가하였다. 교란변수 통제방법에서 다변량 회귀분석을 시행하지 않은 연구 6건에 대해 비뚤림 위험이 높다고 평가하였다. 비뚤림에 대한 판단이 어려운 연구(unclear risk of bias)는 없었다(Figure 2).

3. 감염병 대응 의사의 번아웃 측정 결과

32건의 문헌 중 번아웃 측정도구로 가장 많이 사용된 MBI로 측정한 결과는 다음과 같다(Table 2) [13,16,20-49]. Azoulay 등[20]은 MBI로 측정한 값을 median (interquartile range) 값으로 제시하였다. 정서적 소진 18 (10-29), 비인격화 8 (4-12), 개인적 성취감 35 (29-40)이었다. Buran과 Altin [21]은 MBI로 측정한 값을 남성과 여성으로 나누어 살펴보았고, 결과를 median (min–max)값으로 제시하였다. 정서적 소진에서 남 27 (11-44), 여 31 (11-42)였고, 비인격화는 남 13 (5-20), 여 13 (5-22)였으며, 개인적 성취감은 남 22 (14-32), 여 22 (16-31)이었다. Yao 등[22]은 남성만 제시하였으며, odds ratio (OR)와 95% confidence interval (CI)로 보고하였다. 정서적 소진 0.54 (0.22-1.35), 비인격화 0.62 (0.23-1.68), 개인적 성취감은 평가하지 않았다. Kok 등[23]은 mean±standard deviation (SD)으로 결과를 제시하였으며, 정서적 소진 1.27±0.70, 비인격화 1.30±0.79, 개인적 성취감 4.29±0.68로 보고하였다. Varani 등[24]은 결과를 mean±SD로 제시하였으며, 정서적 소진 13.3±7.6, 비인격화 7.0±4.6, 개인적 성취감 36.5±6.2였다. Tuna와 Ozdin [25]은 성별을 구분하여 mean±SD로 제시하였으며, 정서적 소진에서 남 16.4±7.5, 여 18.7±6.8이었고, 비인격화는 남 6.3±3.6, 여 6.2±3.6, 개인적 성취감에서 남 21.2±4.1, 여 20.9±3.6으로 나타났다. Cravero 등[16]은 정서적 소진과 비인격화 두 가지에 대하여 평가하였다. Ismail 등[26]은 mean±SD로, 정서적 소진에 대해서 남 20.3±2.5, 여 22.0±2.3으로 보고하였다.

Abbreviated-MBI 도구로 측정한 연구는 두 개의 연구가 있으며, mean±SD로 결과를 제시하였다. Khalafallah 등[27]은 정서적 소진 7.86±5.18, 비인격화 5.22±4.65, 개인적 성취감 14.62±3.07이었다. Doherty 등[28]의 연구에서는 정서적 소진 7.05±5.12, 비인격화 2.73±3.46, 개인적 성취감 15.06±2.71로 나타났다. Elhadi 등[29]은 정서적 소진 11.2±5.15, 비인격화 8.55±5.08, 개인적 성취감 12.61±3.89로 보고하였다.

MBI-Human Services Survey for Medical Personnel 도구로 측정한 Asghar 등[30]의 연구는 결과를 mean±SD로 제시하였으며, 정서적 소진 25.29±12.21, 비인격화 16.13±5.63, 개인적 성취감 33.40±7.03으로 확인되었다. Zhizhong 등[31]은 mean±SD로 결과를 제시하였으며, 정서적 소진 26.5±11.6, 비인격화 10.6±6.8, 개인적 성취감 34.2±8.9였다.

MBI-Human Services Survey 도구로 측정한 Treluyer와 Tourneux [32]는 결과를 95% CI로 제시하였으며, 정서적 소진 19.1-28.4, 비인격화 23.5-33.3, 개인적 성취감 20.8-30.3이었다. Park 등[33]은 mean±SD로 결과를 제시하였으며, 정서적 소진 34.92±10.01, 비인격화 10.55±5.69, 개인적 성취감 31.66±8.18로 보고하였다. Abdelhafiz 등[34]은 정서적 소진 20.67±13.34, 비인격화 7.20±6.14, 개인적 성취감 18.53±10.19로 나타났다.

MBI-General Survey 도구로 측정한 Lasalvia 등[35]의 연구는 adjusted OR과 95% CI를 제시하였으며, intensive care units, subintensive care wards for patients with COVID-19, front-line services dealing with patients with COVID-19, non-COVID wards, laboratory diagnostic services, administration에서 각각 정서적 소진, professional efficacy, cynicism을 평가한 결과를 보고하였다.

고찰

이 연구는 전 세계적으로 코로나19로 인한 의사의 번아웃과 관련된 문헌을 종합하고 체계적으로 분석하여 현황을 확인하고자 하였다.

최초 수집된 문헌은 총 465건이었으며, 문헌 선택의 전 과정은 두 명의 연구자에 의해 독립적으로 수행되었으며, 충분한 논의를 통하여 문헌 선택의 기준을 일치시켜 최종적으로 32건을 선정하였다. 최종 선정된 문헌 32건에 대한 질 평가를 시행한 결과 충분한 설명이 기재되어 비뚤림 위험이 낮다고 평가한 연구는 19건이었다. 연구가 진행된 국가는 한국, 미국, 프랑스, 영국, 폴란드, 싱가포르, 사우디아라비아, 대만, 네덜란드, 아일랜드, 파키스탄, 터키, 브라질, 캐나다, 스페인, 이탈리아, 벨기에, 이집트, 리비아, 중국이었다. 의사 전문과목별로 internal medicine, emergency medicine, otolaryngology, neurosurgery, pediatric medicine, palliative medicine, medical oncology, radiation oncology, surgery, plastic surgery, pathology, genetics, radiology로 구성되어 있었다. 연구결과인 번아웃을 측정하기 위해서 14종류의 도구가 사용되었으며, 가장 많이 사용된 도구는 MBI였다.

본 연구의 분석대상 연구설계는 단면연구, survey 형식의 연구가 대부분으로, 전 세계적으로 코로나19로 인한 감염병 대응 의사의 번아웃에 미치는 영향에 관한 연구는 초기단계인 것으로 보인다. 무작위대조시험 연구보다는 관련 요인 및 결과 중심의 연구가 더 많으며, 향후 코로나19 이전과 이후로 의사의 번아웃을 비교하는 연구가 필요하다. 코로나19로 인한 번아웃은 국내 의료진에게도 중요한 문제로 인식되고 있으며, 정책적으로, 실무적으로 의사의 번아웃을 해소하고 예방하기 위한 방안이 필요한 실정이다.

국내에서는 2020년 4월 기준 의사 1,723명을 포함하여 간호사와 간호조무사, 기타 인력 등 총 3,720명이 코로나19에 대응하기 위해 헌신적인 노력을 펼친 것으로 알려졌으며[50], Kim 등[51]은 2020년 11월 기준으로 1,910명의 공중보건의가 코로나19 방역을 위해 파견되었음을 보고하였다. 코로나19 대응을 위해 외부에서 의료인력이 파견되지만, 그 수가 충분하지 않을 뿐만 아니라, 기존 의료인력이 이탈하는 경우도 있다[52,53]. 따라서 감염병 일선 현장에서의 인력부족 문제가 해결되지 않으며, 인력부족으로 인한 의료진의 과로가 심각하다. 이는 번아웃으로 이어질 가능성이 높으며, 코로나19 대응의 지속가능성에 큰 문제를 일으킬 우려가 있다.

코로나19는 변이 바이러스의 발생과 유행으로 인해 종식되는 시점을 예상할 수 없다. 코로나19에 대응하는 의사는 많은 책임감과 스트레스를 느끼고 있을 것이며, 과도한 업무로 인한 심리적 압박감도 상당히 클 것이다. 이에 우리 정부는 의사 및 의료진을 번아웃으로부터 보호하기 위하여 보건복지부 국립정신건강센터가 코로나19 대응 의료진의 스트레스 관리를 지원한 업무협약의 체결을 발표하였다[54].

세계보건기구에 의하면 유럽을 포함한 다수 국가에서 코로나19 관련 업무에 종사하는 전문가들을 위해 정신건강, 육아, 재정 등을 지원하고 있다. 정신건강 지원방안은 주로 상담을 통한 심리적 지원이었다. 영국과 벨기에, 핀란드 등 일부 국가에서는 정신건강 상담서비스의 접근성을 높여 어플리케이션과 온라인 서비스를 통해 사용할 수 있도록 하였다. 예를 들어, 몰타는 정부가 의료진, 경찰 등 특정 직업군을 대상으로 아동보호센터를 직접 개설하였다. 재정적 지원방안을 수립한 국가는 19개였다. 키르기스스탄, 리투아니아, 프랑스, 벨라루스, 리투아니아, 몬테네그로 등의 국가는 코로나 관련 업무에 대한 급여인상, 상여금 등을 지급함으로써 통상적인 급여 이상을 지급한다고 하였다. 캐나다의 경우, 연방정부와 지자체가 필수 의료 종사자의 임금 인상분을 나누어 지불하는 데 합의하였다. 폴란드는 보건의료종사자의 안전을 보호하기 위하여 2020년 4월 29일부터 3개월간 코로나19 환자와 접촉한 경우, 해당 종사자가 한 개소 이상의 의료기관에서 근무하는 것을 금지하기도 하였다[55].

감염병 최일선에서 대응하고 있는 의사 및 의료진의 번아웃으로 인해 방역체계에 구멍이 뚫리지 않도록 더욱 정부에서는 실질적이고 합리적인 지원방안을 마련하는 것이 필요하고, 국민의 건강을 책임지고 있는 의료인에 대한 정책적 배려와 지원을 보다 적극적으로 검토해야 할 것이다.

이 연구는 코로나19가 의사의 번아웃에 미치는 영향을 주제로 한 문헌을 체계적으로 분석하였으나 이 결과를 우리나라 전체 의사인력에 일반화하기에는 제한점이 있을 수 있다. 특히 우리나라의 의사인력은 개원의, 봉직의, 의과대학 교수, 공중보건의(군의관) 등으로 세분화되어 있어 다른 국가의 의사인력과 큰 차이가 있기 때문이다. 그러나 전 세계적으로 코로나19가 의사의 번아웃에 미치는 영향을 확인함으로써, 관련 정책 수립에 기초자료를 제공하는 데 의미가 있을 것이다.
