무증상 평균위험의 성인을 위한 국가 대장암 검진 권고안: 2025년 개정판

Abstract

Purpose: To develop the 2025 update to the Korean colorectal cancer (CRC) screening guidelines by systematically evaluating recent evidence, integrating domestic data, and addressing changes since the 2015 guideline revision, thereby providing an evidence-based standard for clinicians and policymakers.
Methods: A multidisciplinary committee developed the guidelines using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) methodology. The process included formulation of three key questions addressing screening efficacy, diagnostic accuracy, and optimal screening age and interval. A systematic review of international guidelines and primary literature was conducted, yielding 327 eligible studies. In addition, a utility-based analysis using a Markov model was performed to determine optimal screening ages and intervals.
Results: The evidence synthesis identified high-certainty evidence supporting the use of the fecal immunochemical test (FIT) for reducing CRC mortality and moderate-certainty evidence for colonoscopy. Evidence for computed tomographic colonography (CTC) and stool DNA testing was rated as very low certainty. Based on the evidence review and cost-utility analysis, the committee conditionally recommends CRC screening for asymptomatic, average-risk adults aged 45–74 years using either colonoscopy every 10 years or FIT every 1–2 years. CTC and stool DNA testing were not recommended owing to insufficient evidence.
Conclusion: The 2025 Korean Guidelines for Colorectal Cancer Screening present updated, evidence-based recommendations tailored to the domestic healthcare context. By conditionally endorsing both colonoscopy and FIT for individuals aged 45–74 years, these guidelines aim to improve population-level screening effectiveness and reduce the burden of CRC in South Korea.

서론

대장암(colorectal cancer)은 한국에서 중요한 공중보건 문제 중 하나이다. 2024년 국가암등록통계에 따르면, 대장암은 2022년 기준 신규 암 발생 33,158건으로 전체 암 발생의 11.8%를 차지하며, 국내에서 두 번째로 많이 진단된 암이었다[1]. 연령표준화 발생률은 2010년부터 2020년까지 감소하는 추세를 보였으나, 2020년 이후 연평균 5.5%의 증가율로 다시 상승하기 시작하였으며, 최근 발표된 2023년 통계에서도 이러한 증가 추세가 지속되고 있다. 이러한 발생 증가에는 코로나바이러스병-19 (coronavirus disease 2019)로 인한 진단 지연과 신경내분비종양 분류 변경의 영향이 일부 포함되어 있으나, 2023년 기준 대장암은 남성에서 암 사망 원인 3위, 여성에서 2위를 차지하고 있다. 한편, 한국의 대장암 5년 상대생존율은 74.6%로, 미국, 영국, 일본에 비해 높은 수준을 보이고 있으며, 이는 조기 발견과 치료 성과의 향상을 반영하는 지표로 해석된다[13].
한국의 국가대장암검진사업(National Cancer Screening Program)은 2002년 개발된 권고안을 기반으로 2004년부터 도입되었다[4]. 현재 검진 프로그램은 50세 이상 성인을 대상으로 매년 분변면역화학검사(fecal immunochemical test, FIT)를 시행하도록 권고하고 있으며, FIT 양성자의 경우 확진을 위해 대장내시경을 시행하는 2단계 검진 체계를 유지하고 있다. 이전의 주요 권고안 개정은 2015년에 이루어졌으며, 당시 권고안에서는 45–80세 성인을 대상으로 FIT를 권고하고, 대장내시경을 선택적 검진 방법으로 제시하였다[5]. 이후 1차 검진으로서 대장내시경의 적용 가능성을 평가하고 국내 근거를 보완하기 위해, 2019년부터 2024년 3월까지 대규모 국가 단위 대장내시경 시범사업이 수행되었고, 이를 통해 중요한 국내 실증 자료가 축적되었다[6].
국제적으로도 대장암 검진 권고에는 최근 상당한 변화가 나타나고 있다. 미국 예방서비스 태스크포스(US Preventive Services Task Force, USPSTF)와 미국소화기학회(American College of Gastroenterology, ACG)를 포함한 주요 기관들은, 조기 발병 대장암의 증가를 근거로 무증상 평균위험군에서 검진 시작 연령을 기존의 50세에서 45세로 하향 조정하였다[79]. USPSTF는 50–75세 성인에 대해서는 grade A 권고를, 45–49세 성인에 대해서는 grade B 권고를 제시하고 있다. 이들 국제 가이드라인은 대장내시경을 10년마다 시행하거나 매년 FIT를 시행하는 다중 검진 전략을 권고함으로써, 환자와 임상의가 여러 효과적인 검진 방법 중에서 선택할 수 있도록 하고 있다.
이러한 변화 속에서 2015년 한국 대장암 검진 권고안은 여러 측면에서 개정이 필요한 상황에 놓여 있다. 우선, 국내 의료 환경에서는 대장내시경의 접근성과 안전성이 크게 향상되었고, 다양한 검진 기술 또한 지속적으로 발전해 왔다[1013]. 더불어 지난 10여 년간 전 세계적으로 다양한 검진 방법의 효과, 이득 및 위해에 관한 근거가 상당히 축적되었다[8,1416]. 여기에 더해, 국가 대장내시경 시범사업이 완료되면서 실제 한국 의료 현장을 반영한 중요한 자료가 확보됨에 따라, 기존 검진 전략을 최신 근거와 국내 의료 환경에 부합하도록 체계적으로 재평가할 필요성이 제기되었다[17].
이번 권고안 개정의 주요 목적은 한국의 임상의, 공중보건 전문가 및 정책결정자를 대상으로 대장암 검진에 대한 최신 근거 기반 표준을 제시하는 데 있다. 이를 위해 최근의 근거를 체계적으로 검토하고, 검진 방법, 대상 연령, 검진 간격에 대한 명확하고 일관된 권고를 제시하고자 한다. 궁극적으로 본 권고안은 향후 국가암검진 정책 수립의 근거 자료로 활용되어, 국민 건강 수준을 향상시키고 국내 대장암 질병 부담을 감소시키는 데 기여하는 것을 목표로 한다.

방법

권고안 개발 과정 및 위원회 구성

이번에 개정된 임상진료지침은 국립암센터 주관으로 구성된 대장암 검진 권고안 개정위원회에서 개발하였다. 위원회는 대한가정의학회, 대한내과학회, 대한진단검사의학회, 대한예방의학회, 대한병리학회, 대한영상의학회, 대한외과학회 등 주요 국내 전문학회로부터 추천된 전문가들로 구성된 다학제 위원회였다. 위원회는 2015년 대장암 검진 권고안을 최신 의학적 근거와 국내 보건의료 환경의 변화를 반영하여 개정하는 것을 주요 과제로 하였다. 권고안 개발 전 과정은 국제적으로 널리 활용되는 Grading of Recommendations, Assessment, Development and Evaluation (GRADE) 방법론을 적용하여 수행하였으며, 이를 통해 근거 평가와 권고 도출 과정의 체계성과 투명성을 확보하고자 하였다[18,19].

핵심 질문 설정 및 결과 지표 결정

위원회는 권고안의 범위를 PIPOH 프레임워크에 따라 설정하였다. 대상 인구(Population)는 무증상 평균위험군 성인으로 정의하였고, 중재(Intervention)는 대장내시경, FIT, 구아이악 기반 분변잠혈검사(guaiac fecal occult blood test, gFOBT), computed tomography (CT) 대장조영술(CT colonography), 대변 DNA 검사(stool DNA test)를 포함하였다. 적용 대상 전문가(Professional)는 임상의, 보건의료 제공자 및 정책결정자로 설정하였으며, 평가 결과(Outcome)는 이득, 정확도, 위해를, 의료 환경(Health setting)은 1·2·3차 의료기관을 포함하도록 정의하였다.
이러한 범위 설정을 바탕으로, 각 검진 방법의 효과(KQ1), 진단 정확도(KQ2), 그리고 적절한 검진 시작 연령, 종료 연령 및 검진 간격(KQ3)을 평가하기 위한 총 3개의 핵심 질문(Key Questions, KQs)을 도출하였다. 평가 변수의 중요도는 Delphi 기법을 통해 위원회 합의를 거쳐 결정하였으며, 대장암 특이 사망률과 발생률을 핵심 결과 지표(critical outcomes)로 선정하였다. (1) 핵심 질문 1 (KQ1): 무증상 평균위험군 성인에서, 각각의 검진 방법(대장내시경, FIT, gFOBT, 분변 DNA 검사, CT 대장조영술)은 비검진군에 비해 대장암 특이 사망률 또는 발생률을 감소시키는가? (2) 핵심 질문 2 (KQ2): 무증상 평균위험군 성인에서, 대장암 발견을 위한 각각의 검진 방법의 진단 정확도(즉, 민감도와 특이도)는 어떠한가? (3) 핵심 질문 3 (KQ3): 무증상 평균위험군 성인에서, 각각의 검진 방법의 효과적인 검진 주기와 대상 연령 범위는 무엇인가?

체계적 문헌고찰 및 문헌 검색

근거 검토는 이전 권고안의 문헌 검색 시점인 2013년 10월 이후 발표된 고품질 임상진료지침을 대상으로 한 체계적 검색으로 시작하였다(Suppl. 1). PubMed, Guideline International Network, USPSTF 데이터베이스를 활용하여 총 11개의 관련 가이드라인을 선별하였고, Appraisal of Guidelines for Research and Evaluation (AGREE) II 도구를 이용해 질 평가를 수행하였다. 그 결과, 2021년 USPSTF 권고문을 핵심 근거 문서로 선정하였다.
이후 선정된 USPSTF 권고문 발표 이후의 1차 연구를 확인하기 위해, 2024년 1월 21일 Ovid-Medline, Ovid-Embase, Cochrane Library, KoreaMed 데이터베이스에서 추가 문헌 검색을 시행하였다(Suppl. 2). 총 10,681편의 문헌이 검색되었으며, 독립적인 검토자들이 다단계 선별 과정을 거쳐 최종적으로 327편의 연구를 근거 종합에 포함하였다(Figure 1).

근거 질 평가 및 권고안 도출

포함된 연구의 방법론적 질과 비뚤림 위험은 연구 설계에 따라 적절한 도구를 사용하여 평가하였다. 무작위배정 비교연구(randomized controlled trial, RCT)는 risk-of-bias 2 (RoB 2) 도구를 적용하였고[20], 비무작위 연구(코호트 연구)는 Risk of Bias Assessment Tool for Nonrandomized Studies-2 (RoBANS-2)를 사용하였다[21]. 진단 정확도 연구의 경우 Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2)를 적용하였으며[22], 체계적 문헌고찰 및 메타분석은 A Measurement Tool to Assess Systematic Reviews-2 (AMSTAR-2)를 이용하여 평가하였다[23].
각 평가 결과는 GRADE 프레임워크에 따라 근거의 확실성을 높음, 중등도, 낮음, 매우 낮음의 네 수준으로 분류하였다. 최종 권고 도출 과정에서는 GRADE Evidence to Decision 프레임워크를 활용하였으며[24], 이 과정에서 각 검진 방법의 이득과 위해의 균형, 근거의 확실성, 환자의 가치와 선호, 자원 소요를 종합적으로 고려하였다. 위원회는 이러한 논의를 바탕으로 투표를 통해 권고의 방향과 강도(강한 권고 또는 조건부 권고)를 결정하였다.
핵심 질문별 분석에서는, KQ1과 KQ2에 대해 대장내시경, FIT, gFOBT, CT 대장조영술, 분변 DNA 검사 등 5가지 검진 방법을 대상으로 효과(대장암 특이 사망률 및 발생률 감소)와 위해(합병증, 위양성 등), 그리고 진단 정확도(민감도, 특이도)를 체계적으로 평가하였다.
검진 시작 연령, 종료 연령 및 검진 간격을 다룬 KQ3에 대해서는 체계적 문헌고찰과 효용 기반 분석을 결합한 이중 접근법을 적용하였다. 다양한 검진 방법, 연령 및 간격 조합에 따른 장기 효과와 효율성을 비교하기 위해 Markov model을 구축하였으며, 주요 효율성 지표로는 질보정생존연수(quality-adjusted life years, QALYs)를 사용하였다. 증분 QALY당 추가 검진 횟수를 산출하여 가장 효율적인 검진 전략을 도출하였고, 이러한 모형 분석 결과를 문헌고찰 결과와 함께 종합하여 검진 일정에 대한 권고의 근거로 활용하였다.
최종 권고안은 관련 학술단체 20곳의 외부 검토와 공청회를 거쳐 임상적 타당성과 사회적 합의를 반영하여 확정하였다.

결과

국제 가이드라인 검토

이번 권고안 개정의 맥락을 파악하기 위해 주요 국제 가이드라인 16편을 검토하였다(Table 1). 그 결과, 대장암 검진 시작 연령을 기존의 50세에서 45세로 하향 조정하는 국제적 추세가 뚜렷하게 확인되었으며, 이러한 변화는 USPSTF (2021), ACG (2021), 미국암학회(American Cancer Society, 2024) 등 주요 기관에서 이미 반영되고 있다. 이들 가이드라인에서 1차 권고 검진 방법(Tier 1)은 주로 10년 간격의 대장내시경과 연 1회의 FIT로 구성되어 있었다. 무증상 평균위험군을 대상으로 45세부터 검진을 시작하도록 한 2025년 국립암센터 권고안은 이러한 최신 국제적 합의와 일치한다.

대장내시경의 효과, 정확도 및 위해 (KQ1-1)

대장내시경에 대한 근거는 1편의 대규모 무작위배정 비교연구(RCT)와 3편의 코호트 연구를 기반으로 평가하였다(Tables 2, 3). 해당 RCT에서는 검진 초청군에서 대장암 발생률이 17% 감소하였으나(상대위험도[relative risk, RR], 0.83; 95% 신뢰구간[confidence interval, CI], 0.72–0.96), 대장암 특이 사망률 감소는 통계적으로 유의하지 않았다(RR, 0.92; 95% CI, 0.69–1.21). 반면, 코호트 연구를 포함한 메타분석에서는 대장암 발생률(RR, 0.68; 95% CI, 0.65–0.72)과 사망률(RR, 0.37; 95% CI, 0.26–0.53) 모두에서 유의한 감소 효과가 확인되었다.
주요 위해로는 중대한 합병증이 보고되었으며, 통합 추정 결과 주요 출혈은 0.18%, 천공은 0.05%로 나타났다. 이러한 근거를 종합하여 대장내시경에 대한 전체 근거의 확실성은 중등도(moderate)로 평가하였다.

FIT 검사의 효과, 정확도 및 위해(KQ1-2, KQ2-2)

FIT에 대해서는 포함 기준을 충족하는 RCT는 확인되지 않았다. 그러나 대규모 코호트 연구를 포함한 메타분석에서 대장암 발생률은 33% 감소(RR, 0.67; 95% CI, 0.66–0.69), 대장암 사망률은 53% 감소(RR, 0.47; 95% CI, 0.45–0.49)하는 것으로 나타났다. 주요 위해로는 위양성 결과가 평균 6–8%에서 발생하였으며, 이로 인해 추가적인 대장내시경 검사가 필요해져 수검자의 불안이 증가할 수 있는 것으로 평가되었다.
진단 정확도 분석에서 절단값(cutoff)을 20 µg Hb/g으로 설정했을 때, 통합 민감도는 0.74, 특이도는 0.94로 나타났다. 이러한 결과를 바탕으로 FIT에 대한 전체 근거의 확실성은 높음(high)으로 평가하였다.

이외 검진 방법에 대한 근거(KQ1-3, 4, 5, KQ2-3, 5)

gFOBT의 경우, RCT에서 대장암 사망률을 약 15% 감소시키는 효과가 확인되었으나, 현재 국내에서는 해당 검사에 필요한 시약이 생산 또는 수입되지 않아 실제 적용이 어려운 것으로 판단하였다. CT 대장조영술과 분변 DNA 검사에 대해서는 대장암 발생률 또는 사망률 감소를 직접적으로 입증한 RCT나 코호트 연구가 확인되지 않았다.
또한, 국제 연구에서 주로 평가된 다중표적 분변 DNA 검사(multi-target stool DNA)는 국내에서 상용화된 단일표적 분변 DNA 검사와 검사 특성이 근본적으로 달라, 기존 근거를 국내 상황에 그대로 적용하기에는 한계가 있었다. 이에 따라 CT 대장조영술과 분변 DNA 검사에 대한 근거의 확실성은 모두 매우 낮음(very low)으로 평가하였다.

최적 검진 연령 및 주기: 효용 분석 (KQ3)

KQ3에 대해서는 Markov model을 활용하여 다양한 검진 전략의 효용(QALYs)과 효율성(증분 QALY당 추가 검진 횟수)을 평가하였다. 대장내시경 분석 결과(Figure 2A), 5년 간격 및 넓은 연령 범위 전략(예: S5, 45세–무제한)은 가장 높은 QALYs (1,000명당 10,628)를 보였으나, 검진 부담 또한 가장 컸다(1,000명당 8,398회).
효율성 분석에서는 15년 간격 전략(S26, 45–74세)이 가장 효율적인 전략으로 나타났으며, 1,000명당 10,626 QALYs와 1,923회의 대장내시경 검사를 보였다. 이는 QALY가 가장 낮은 전략(S32, 15년 간격, 55–69세) 대비 증분 검진 횟수당 0.0099 QALYs 증가에 해당하였다. 다만 위원회는 임상적 효과성, 효율성, 국민 수용성을 종합적으로 고려할 때 10년 간격 전략(S16, 45–74세)이 가장 균형 잡힌 선택이라고 판단하였다.
FIT 분석(Figure 2B)에서는 1년 간격 전략(예: S5, 45세–무제한)이 가장 높은 QALYs (1,000명당 10,627)를 보였으나, 추적 대장내시경 검사 수 역시 가장 많았다(1,000명당 2,675.9회). 효율성 분석 결과, 2년 간격 전략(S16, 45–69세)이 가장 효율적인 전략으로 확인되었으며, 1,000명당 10,624 QALYs와 815회의 대장내시경 검사를 보였다. 이는 QALY가 가장 낮은 전략(S26, 2년 간격, 55–69세) 대비 증분 검진 횟수당 0.0221 QALYs 증가에 해당하였다.
이러한 분석 결과를 바탕으로 위원회는 FIT를 1–2년 간격으로 시행하는 전략이 건강 효과를 극대화하면서 자원 소요를 관리하는 데 가장 적절하다고 판단하였다.

연령별 대장내시경 관련 합병증 발생률

총 28,815,919건의 대장내시경 자료를 분석한 결과, 검사 후 1주 이내 전체 합병증 발생률은 0.47%였으며, 이 중 출혈은 0.42%, 천공은 0.03%를 차지하였다(Table 4). 출혈은 모든 연령대에서 가장 흔한 이상반응이었다. 합병증 발생률은 연령 증가에 따라 뚜렷하게 상승하는 양상을 보였으며, 20세 미만에서 가장 낮은 발생률(0.27%)을 보인 반면, 85세 이상에서는 1.97%로 가장 높았다. 특히 85세 이상에서는 출혈률이 1.75%로 합병증 증가의 주요 원인이었다. 관찰 기간을 1개월로 확장했을 때 전체 합병증 발생률은 0.49%로 소폭 증가하였으며, 85세 이상 연령군에서의 위험은 2.10%로 가장 높게 유지되었다.

권고안 요약

효과, 정확도 및 위해, 비용-효용에 대한 근거 종합을 바탕으로 위원회는 최종적으로 4가지 권고안을 도출하였다. (1) 45–74세 무증상 성인을 대상으로 10년 간격의 대장내시경 검사를 통한 대장암 선별검사를 권고한다. (2) 45–74세 무증상 성인을 대상으로 1–2년 간격의 면역화학적 대변잠혈검사(FIT)를 통한 대장암 선별검사를 권고한다. (3) 무증상 성인을 대상으로 CT 대장조영술검사를 통한 대장암 선별검사를 시행하는 것은 이득과 위해의 크기를 비교 평가할 만한 근거가 불충분하다. (4) 무증상 성인을 대상으로 분변 DNA (stool DNA) 검사를 통한 대장암 선별검사를 시행하는 것은 이득과 위해의 크기를 비교 평가할 만한 근거가 불충분하다.

고찰

이번 2025년 한국 대장암 검진 권고안 개정은 국내 대장암 역학의 변화, 검진 기술의 발전, 그리고 최근 10여 년간 축적된 근거를 종합적으로 반영하였다. 2015년 권고안 발표 이후 10년이 경과한 시점에서, 이번 개정의 주요 변화는 검진 종료 연령을 80세에서 74세로 조정하고, 무증상 평균위험군 성인을 대상으로 FIT와 대장내시경을 모두 조건부로 권고한 점이다. 또한 FIT와 gFOBT에 대한 권고를 명확히 구분하여 정리하였으며, 국내 임상 환경을 고려하여 gFOBT는 권고 검진 방법에서 제외하였다. 이러한 개정은 한국의 의료 환경에 부합하는 최적의 근거 기반 대장암 검진 전략을 제시하고, 궁극적으로 대장암 질병 부담을 감소시키는 데 목적이 있다.
이번 권고안은 무증상 평균위험군 성인을 대상으로 FIT (1–2년 간격)와 대장내시경(10년 간격)을 모두 검진 선택지로 조건부 권고한다. FIT는 대장암 발생률과 사망률 감소에 대해 높은 확실성의 근거가 확인되었으며, 비침습적 검사라는 특성과 상대적으로 낮은 비용으로 인해 수검자의 선호도와 프로그램 수용성이 높은 장점을 가진다[25,26]. 반면, 대장내시경은 대장암 발생률 감소에 대해 중등도 확실성의 근거를 보였으며, 전암 병변인 선종을 발견하고 제거함으로써 암 발생을 예방할 수 있다는 고유한 장점을 가진다[27]. 그러나 침습적 검사라는 특성상 출혈이나 천공과 같은 드물지만 중대한 합병증의 위험이 있으며, 자원 소모 또한 상대적으로 크다[28]. 위원회는 이러한 이득과 위해의 차이, 근거의 확실성, 그리고 환자 선호도의 다양성을 고려하여 두 검진 방법을 모두 제시하고, 임상의와 환자 간의 공동 의사결정(shared decision-making)을 통해 적절한 검사를 선택하는 것이 중요하다고 판단하였다.
대장내시경의 10년 간격 권고는 비용-효용 분석 결과, 국제 가이드라인, 그리고 국내 보건의료 환경을 종합적으로 고려하여 결정되었다. 모형 분석에서는 15년 간격 전략이 가장 높은 비용-효용성을 보였으나, 10년 간격 전략은 추가 검사 수 증가 대비 증분 QALY 향상이 뚜렷하여 공중보건적 관점에서 보다 합리적인 선택으로 평가되었다. 또한 10년 간격은 여러 국제 가이드라인에서 이미 표준으로 권고되고 있는 전략이다[7,9]. 국내의 높은 내시경 접근성, 숙련된 전문 인력, 그리고 국민의 검진 선호도를 고려할 때, 10년 간격은 효과성과 수용성을 균형 있게 충족하는 전략으로 판단되었다[10]. 공청회 과정에서 5년 간격에 대한 의견도 제시되었으나, 무증상 평균위험군을 대상으로 한 비용-효용성 근거는 현재로서는 충분하지 않은 것으로 평가되었다.
FIT의 1–2년 간격 권고 역시 비용-효용 분석 결과를 근거로 한다. 분석 결과, 2년 간격 전략은 가장 높은 비용-효용성을 보였으며, 1년 간격 전략은 QALY를 최대화하는 전략으로 확인되었다. 현재 국가암검진사업이 1년 주기로 운영되고 있다는 점과, 간격 연장 시 수검률 또는 효과 감소에 대한 우려를 함께 고려하여, 위원회는 1–2년 간격이라는 유연한 권고를 제시하였다[17]. 이는 기존 검진 체계와의 연속성을 유지하면서도, 2년 간격 검진의 높은 비용-효용성을 반영한 균형 잡힌 접근으로 볼 수 있다.
검진 종료 연령은 대장내시경 관련 합병증 위험이 급격히 증가하고 순이득이 감소하는 시점을 고려하여 74세로 설정하였다. 국민건강보험 청구자료 분석에서도 75세 이상에서 합병증 발생률이 유의하게 증가하는 것이 확인되었다. 공청회에서는 기대수명 증가와 건강한 노화 추세를 근거로 80세까지 검진을 유지해야 한다는 의견도 제시되었으나, 위원회는 인구 집단을 대상으로 하는 국가 검진 프로그램의 안전성을 우선적으로 고려하여 74세를 종료 연령으로 설정하였다. 다만, 이 권고안은 75세 이상에서도 개인의 건강 상태, 기대수명, 환자 선호를 종합적으로 고려하여 임상의와 상담 후 검진 지속 여부를 개별적으로 결정할 수 있음을 명시하고 있다.
이번에 개정된 권고안은 gFOBT, CT 대장조영술, 분변 DNA 검사에 대해서는 권고를 제시하지 않았다. gFOBT는 대장암 사망률 감소 효과가 입증된 검사이나, 현재 국내에서는 필수 시약의 생산 또는 수입이 이루어지지 않아 실제 적용이 불가능하다. CT 대장조영술과 분변 DNA 검사의 경우, 대장암 발생률 또는 사망률 감소를 직접적으로 입증한 근거가 부족하며, 근거의 확실성 또한 매우 낮은 수준으로 평가되었다. 특히 분변 DNA 검사는 국제 연구에서 평가된 다중표적 검사와 국내에서 주로 사용되는 단일표적 검사 간의 차이로 인해 기존 근거를 국내 상황에 적용하는 데 한계가 있다[29]. 따라서 이들 검진 방법은 향후 추가 연구를 통해 근거가 축적되기 전까지는 인구 집단 검진으로 권고하기 어렵다고 판단하였다.

결론

요약하면, 2025년 한국 대장암 검진 권고안은 GRADE 방법론에 기반하여 체계적 문헌고찰, 메타분석, 국내 자료 분석, 비용-효용 분석, 이해관계자 의견을 종합적으로 반영하여 개발되었다. 본 권고안의 강점은 근거 중심 접근과 국내 보건의료 환경 및 비용-효용성을 함께 고려하였다는 점에 있다. 반면, 일부 권고는 국제 자료에 의존하고 있어 국내 연구 축적의 필요성이 남아 있다. 향후에는 이번 개정 권고안의 효과적인 이행과 함께, 질 관리 체계 구축, 검진 성과 모니터링, 검진 접근성 격차 해소, 환자 중심 정보 제공 및 의사결정 지원 강화, 그리고 새로운 근거에 따른 주기적 개정이 지속적으로 이루어져야 할 것이다.
